The Role Are you a passionate Social Worker who is committed to supporting vulnerable young parents and their children?
We have an opportunity for an experienced Social Worker to join our Young Family Support Program in Morphett Vale.
This is a part-time position (0.9 FTE) available until end of May ****.
This is an exciting opportunity to work within a supportive and inclusive team, delivering strengths-based, trauma-informed case management to young women who are pregnant and/or parenting.
About the Role Reporting to the Senior Manager, you will provide intensive case management support to young women aged up to 25 years and their children who are engaged with our outreach program.
Working within the Young Family Support Program service model and Case Management Guidelines, you will partner with clients to develop individualised support plans that address their unique goals and needs.
You will support young families across a range of areas, including: Housing and accommodation stability Independent living skills Parenting support and child wellbeing Mental health and emotional wellbeing Alcohol and other drug-related issues Community engagement and social connection Access to education, employment,



and support services This role requires a compassionate and client-centred approach, recognising the impact of homelessness on young parents and children while empowering families to build resilience, independence, and lasting community connections.
Key Responsibilities As a Social Worker, you will: Deliver high-quality, trauma-informed case management to young women and their children.
Work collaboratively with the Senior Practitioner and support staff to implement the Young Family Support Program service model.
Develop, implement, and review individual case plans in partnership with clients.
Support clients to build capacity, strengthen family relationships, and achieve their goals.
Apply evidence-informed practice, including trauma-responsive care, attachment theory, lifespan development, grief and loss, and strengths-based approaches.
Participate in allocation, case management, and multi-agency case conferencing meetings.
Build and maintain strong relationships with government, community, and specialist service providers.
Coordinate referrals and advocate for clients to access appropriate services and supports.
Maintain accurate client records, case notes,



and H2H data in accordance with organisational and legislative requirements.
Respond to the day-to-day needs of young women and their children in a flexible and responsive manner.
Fulfil child protection responsibilities, including mandatory reporting requirements.
Promote a positive, collaborative, and supportive team culture.
Complete Worker Safety Risk Assessments and utilise the Worker Safety Application (WSA) where required.
Attend team meetings, professional development activities, and contribute to continuous service improvement.
Essential Criteria Degree in Social Work Ability to achieve effective client outcomes through quality case management Experience working with vulnerable young people with complex needs and an understanding of the impact of childhood trauma, attachment theory and adolescent development Knowledge of the homelessness sector and intersecting services and agencies Proficient in Microsoft Office Strong written and verbal communication skills See attached Position Description for full details.
