Amplar Health is expanding its home-based care services in Australia. We are seeking an Enrolled Nurse Care Partner to deliver person-centred care and coordinate services for consumers in their homes, supported by a Senior Care Partner.
You'll work with a multidisciplinary primary care network to promote wellbeing, independence and meaningful daily living. You will assess, plan and implement tailored care, monitor wellbeing, and engage families to maximise quality of life.
