Location: Bowen, Proserpine and Surrounding Areas travel required between office locations
Remuneration: 53.55 + 12% Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowances and Travel Reimbursement (KM’s) + Employee discounts

About the role:
As a Care Partner, your role is to ensure that our clients receive person-centred, high-quality care by overseeing all care planning and coordination of services. Working closely with the clinical teams to achieve the best care outcomes for each client from their Aged Care Service funding through activities that include:
Develop and review care plans in collaboration with the client and multidisciplinary care team
Manage budgets and facilitate multidisciplinary care activities
Liaise with clinical teams to monitor and respond to changing needs of complex community clients
Continuously monitor, review and evaluate care plans, services and outcomes
Assist clients in navigating the care system and linking them to necessary resources where required
With no two days ever the same. You’ll spend the majority of your day on the road, engaging with clients within the community.

Role requirements:
Certificate IV in a Health or Community Service discipline Essential
Current Australian Driver’s Licence Essential
Roadworthy and insured (Third Party Property minimum) vehicle to use for work Essential
Demonstrated understanding and sound knowledge of client centred, community-based care services delivery
Demonstrated ability to liaise with the clinical and care team and to provide coordinated care and exercise professional judgment within prescribed areas.
Demonstrated experience and knowledge in Community Funding streams and programs
Excellent communication and clinical problem-solving skills
Confidence in managing your time effectively, including an ability to work autonomously.
Confidence to satisfy the relevant probity checks required by legislation or policies Essential
