Ozcare is seeking an experienced and passionate Care Partnering Lead to provide leadership and support to our Care Partner team servicing the Gladstone region. This permanent part-time opportunity offers the chance to play a key role in driving service excellence, supporting team development, and contributing to the growth of our Home Care Package services while working closely with a remote team based in Gladstone.

Your Role

Reporting to the Clinical & Care Partnering Manager, you will provide leadership, coaching and operational support to a team of Care Partners, ensuring the delivery of high-quality, client-focused care coordination services. You will play an integral role in supporting service growth, maintaining compliance, driving continuous improvement initiatives, and fostering strong relationships with clients, families, healthcare professionals and community stakeholders.

As a Care Partnering Lead you will:

- Based at our Rockhampton Branch you will lead, coach and support a remote Care Partner team servicing the Gladstone region

- Ensure care coordination services are delivered effectively, efficiently and in line with organisational standards and client expectations

- Support Care Partners to assess, plan, coordinate and review services that promote client independence, wellbeing and choice

- Provide guidance and oversight in the development and management of Home Care Package budgets, quotes and care plans

- Foster strong relationships with clients, families, referrers and key stakeholders to support positive client outcomes

- Monitor service quality, customer feedback and performance outcomes, implementing improvement initiatives where required

- Provide supervision, performance support and professional development opportunities for direct reports





- Lead problem-solving and escalation management relating to client services and care delivery

- Support compliance with aged care legislation, quality standards, organisational policies and continuous improvement requirements

- Monitor reporting, documentation, billing processes and data integrity to ensure accuracy and compliance

- Support local service growth initiatives through networking, stakeholder engagement and community promotion

- Work collaboratively with multidisciplinary teams to ensure seamless and coordinated service delivery

Role Requirements:

- Demonstrated experience in community aged care, home care packages, care coordination, case management or a related health/community services environment

- Proven leadership experience with the ability to coach, mentor and support team members

- Strong knowledge of Home Care Packages,



client-centred care and service delivery frameworks

- Experience managing client budgets, service plans and compliance requirements

- Highly developed verbal, written and interpersonal communication skills with the ability to engage a broad range of stakeholders

- Demonstrated ability to build strong relationships and collaboratively work across multidisciplinary teams

- Strong organisational skills with the ability to manage competing priorities and meet deadlines

- Tertiary qualifications and/or relevant experience in a health, aged care or community services related discipline (e.g. Health and Human Services)

- Intermediate computer skills including Microsoft Office applications and client management systems

- Current First Aid and CPR Certificate

- Current National Police Check (valid within the past 3 years) or a willingness to obtain one

- Current Queensland Driver Licence

- Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) and Pertussis (Whooping Cough)

- To be considered, candidates must possess the availability to work a recurring roster of Monday to Friday (8:30am - 3:00pm)
