Position Description

This practice is seeking an Aboriginal Health Worker or Aboriginal Health Practitioner to join its multidisciplinary primary health care team.

The successful candidate will deliver culturally safe, person-centred primary health care and support clients to access services, engage with their health care and achieve improved health outcomes.

Working alongside General Practitioners, Nurses, Allied Health Professionals and support staff, the role contributes to health assessments, chronic disease management, care planning, health promotion, clinical support and community engagement activities.





Key responsibilities include:

- Assisting with health assessments, including 715 Health Checks
- Supporting chronic disease management, care planning and follow-up activities
- Assisting clinical staff with procedures and treatment room activities
- Supporting clients to access health services and navigate the healthcare system
- Promoting culturally secure, respectful and person-centred care
- Participating in health promotion, education and preventative health initiatives
- Maintaining accurate clinical records and documentation
- Contributing to quality improvement and continuous improvement activities
- Working collaboratively within a multidisciplinary healthcare team

Recently qualified Aboriginal Health Workers and Health Practitioners, as well as those nearing completion of their qualification, are encouraged to apply.

Position Requirements

- Must identify as an Aboriginal and/or Torres Strait Islander person
- Hold, or be nearing completion of, a Certificate III in Aboriginal and Torres Strait Islander Primary Health Care; or
- Hold a Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner)




- Experience delivering primary health care services or a strong desire to build a career in Aboriginal and Torres Strait Islander primary health care
- Understanding of health issues impacting Aboriginal and Torres Strait Islander people and communities
- Strong communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Strong organisational skills and ability to manage competing priorities
- Current Open C Class Driver Licence
- Current CPR certification
- Hold, or be able to obtain, a Blue Card and National Police Check

Desirable:

- Experience within an Aboriginal Medical Service or Aboriginal Community Controlled Health Organisation
- Experience supporting chronic disease management and health assessments
- Experience using Best Practice or similar clinical software
- Additional training in health promotion, preventative health or community engagement
- Previous experience in health, aged care, disability or community services
