The Tasmanian Department of Health is seeking a Clinical Nurse Specialist – Discharge Planner to support protected, timely discharges in the Mental Health Inpatient Unit at Royal Hobart Hospital.

You will work with multidisciplinary teams to develop recovery-oriented discharge plans, coordinate with NDIS providers and accommodation services, and ensure transitions to the least restrictive care environment. RN with NMBA registration required.
