We are currently seeking a Registered Nurse or Enrolled Nurse to join our team at Kedron Medical Centre. This is an exciting opportunity to play a key role in Chronic Disease Management, working closely with GPs and patients to improve health outcomes through preventative care and ongoing support.

About the Role
In this role, you will:

Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
Support patients in managing chronic health conditions and achieving positive health outcomes
Conduct health assessments, including preventative health and chronic disease screening activities
Work closely with GPs, specialists and the wider multidisciplinary healthcare team
Provide patient education, coaching and ongoing support to encourage positive health behaviours
Coordinate referrals to allied health professionals and external healthcare providers
Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis where appropriate
Utilise tools such as POLAR, PenCAT and HotDocs to identify eligible patients
Maintain accurate clinical records and appointment administration




Support preventative health initiatives and proactive patient outreach programs

About You
To be successful in this role, you will have:

Current AHPRA registration as a Registered Nurse or Enrolled Nurse
Previous experience in Chronic Disease Management highly regarded
Experience in General Practice or Primary Healthcare preferred
Strong communication and interpersonal skills
Excellent organisational and time management abilities
A proactive, patient-focused approach to care
The ability to work both independently and collaboratively within a team
A passion for preventative healthcare and improving patient outcomes
