About the Role
Make a meaningful difference to the care of older people as Nurse Consultant, Care of Older Persons. You’ll lead and coordinate person-centred care,
guide complex discharge planning and work across teams and services to support safer outcomes and help avoid unnecessary hospital stays.
In this role, you will:
Lead expert nursing care for older people with complex needs
Coordinate assessment, referrals and discharge plans
Develop and review individual care plans with the wider team
Build strong links with residential and community care providers
Support staff to grow aged care knowledge and capability
Drive evidence-based practice, quality improvement and better care outcomes
About You
You’re a compassionate and collaborative Registered Nurse who brings sound clinical judgement, strong communication and a genuine commitment to improving care for older people. You’ll be comfortable working across teams, sharing your expertise and helping others deliver their best care.
To be successful in this role you will:
Hold current or eligible NMBA registration and practising certificate
Bring strong clinical nursing experience and sound judgement
Communicate with confidence and build trusted relationships
Thrive in multidisciplinary teams and manage competing priorities
Lead change, grow staff capability and champion consumer care
