About the Service The SPARK Child and Family Wellbeing Program is a Primary Health Network (PHN) funded initiative providing early intervention mental health and wellbeing support to children aged 5-11 years and their families in Canterbury.

The program delivers child-centred and family-focused support that promotes positive mental health, strengthens family relationships, builds parenting capacity and improves outcomes for children experiencing vulnerability or emerging wellbeing concerns.

Working as part of a multidisciplinary team, the program provides coordinated and accessible support through community-based, home-based, school-based and office-based service delivery.

About the Role

As the Care Coordinator - Child and Family Wellbeing, you will be the central point of support for children and families experiencing developmental, behavioural, emotional, social, health or wellbeing challenges.

You will:

- Build strong and trusting relationships with children and families.
- Conduct assessments and identify child and family strengths, needs and goals.
- Develop and coordinate family-centred support plans.
- Support families to navigate health, education and community service systems.




- Facilitate referrals and connections to appropriate services and supports.
- Collaborate with schools, health professionals, GPs, paediatric services and community organisations.
- Advocate for children and families to ensure equitable access to support.
- Participate in multidisciplinary case discussions and coordinated care planning.
- Maintain accurate documentation, case records and outcome reporting.
- Deliver services across centre-based, home, school and community settings.

About You To be successful in this role, you will have:

- A tertiary qualification in Social Work, Psychology, Counselling, Developmental Education, Community Services, Occupational Therapy, Human Services or a related discipline.
- Experience in care coordination, case management, family support or child and family services.
- Sound knowledge of child development, attachment theory, trauma-informed practice and child mental health.
- Experience facilitating individual and group-based therapeutic programs.
- Strong skills in engagement, assessment, therapeutic planning and intervention.
- Ability to build positive therapeutic relationships with children, families and carers.
- Excellent communication, collaboration and stakeholder engagement skills.
- Experience working with culturally and linguistically diverse communities.
- A commitment to culturally responsive, child-safe and family inclusive practice.
- Current NSW Driver Licence.
- Current Working with Children Check.
- Australian work rights.
