About the role

We have an exciting opportunity for a highly experienced Nurse to oversee all clinical aspects. You will oversee and manage client care for clients, to ensure high-quality outcomes and a best-practices approach to their care. This will include initially conducting thorough intake assessments to ensure that our facilities and capabilities are in alignment with clients' needs, developing comprehensive care plans, as well as monitoring and adjusting client care on an ongoing basis.

The Clinical

Coordinator is responsible for managing the day-to-day clinical ascpect of services to ensure an efficient, effective, and streamlined consumer delivery within the context of specialised disability services and community care.

The Clinical

Coordinator is expected to provide leadership to a team of Team Leaders, and Support Workers, through clinical team meetings. You will ensure the highest standard of care is delivered across our group homes. Using your leadership and clinical expertise you will manage the clinical outcomes and regulatory compliance whilst building a team of happy, kind, trained and highly motivated staff. You will have responsibility for compliance, quality principles and all outcomes from both internal and external audits.

The Clinical

Coordinator will identify quality improvement initiatives that will strengthen the consumer delivery experience and that will ensure all programs and services are cost effective and efficient.





Key responsibilities

- Coordinate aspects of care for our clients - from completing comprehensive intake assessments and developing care plans.
- Where need be, to attending medical appointments with clients to advocate on their behalf and ensuring actions are undertaken from appointments.
- Develop solid client relationships to establish trust, understand client needs and preferences, regularly reassessing care, and support plans to ensure they are aligned with changing needs.
- Ensure adherence to established service agreements and clinical best practice standards, including clinical documentation, forms, registers and assessments.
- Drive clinical excellence through regular monitoring, action planning and execution on clinical outcomes and targets (KPIs)
- Ensure proper medical care is promptly received (e.g., appointments are scheduled with allied health care professionals) and outcome followed through
- Respond to Incident Reports and client or family complaints, promptly resolving issues or escalating when needed.
- Participate in team meetings to ensure client care and medical processes (medication management, wound care, etc.) are clearly understood by all, and provide training as necessary.




- Ensure high-quality standards of care in the homes, from continuity of care to secure client services.
- Ensure PBS for clients are followed, updated appropriately, monitored and reported to NDIS and management promptly including presence of medication purpose form
- Carry out all clinical aspects of onboarding (e.g., understanding the care plan, safe medication administration, de-escalation and physical management techniques)

About you

- Current AHPRA Nursing registration.
- Minimum 3 years experience in Disability, Community or Aged Care.
- Proven experience in Disability, Aged Care, preferably in residential and / or community setting in a clinical role.
- Demonstrated experience, knowledge and understanding of relevant legislation including NDIS standards, Accreditation Standards, Accountability Principles as well as restrictive practice reporting experience assessment, care planning, and validation procedures.
- Advanced clinical leadership experience
- Demonstrated knowledge of contemporary nursing theory and practice in relation to the care of complex, high and low care residents in a residential setting.
- Excellent Understanding and management of the NDIS
- Current National Police Certificate
- Current Working with Children Check
- Experience in strong leadership and/or managerial positions
