About the Role

As a Clinical Nurse Specialist Grade 2 - Chronic Care - Community Health, you will play a pivotal role in delivering specialised nursing care to patients with chronic and complex health needs within a community setting. Working both independently and collaboratively within a multidisciplinary team, you will provide clinical leadership, support service improvement initiatives, and contribute to positive patient outcomes through evidence-based practice.

This role offers the opportunity to utilise your advanced clinical knowledge while mentoring and coaching nursing staff, fostering professional development, and supporting a culture of continuous improvement. You will be responsible for coordinating care, engaging with a range of stakeholders, and using effective communication and technology systems to ensure safe, person-centred care delivery across the community.

About You

You are a passionate and experienced Registered Nurse with current registration through the Nursing and Midwifery Board of Australia and relevant post-registration qualifications. With extensive experience in your clinical specialty, you bring a strong commitment to delivering high-quality patient care and improving outcomes for people living with chronic conditions.

You thrive working autonomously while also valuing collaboration and partnership within multidisciplinary teams.



A natural leader, you enjoy mentoring and supporting others, sharing your expertise to help develop nursing capability and strengthen team performance. Your excellent communication skills, combined with your ability to embrace innovation and drive service improvements, enable you to influence positive change and deliver outstanding care.

What you'll be doing To provide advanced and complex patient care within their area of specialty that reflect current standards of nursing practices and Mid North Coast Local Health District policies and procedures.
