Make a meaningful difference in people's recovery journey Western District Health Service is seeking a compassionate and motivated Community Social Worker to join our Mental Health team.

This rewarding role provides support to patients transitioning from hospital back into their homes and community, helping to improve health, wellbeing and recovery outcomes. Working within a recovery-focused model, you will provide counselling, advocacy, care coordination and practical support to patients, families and community members navigating complex health and social systems.

Based in Hamilton, this role may also include outreach across the Southern Grampians region, supporting individuals to access services and remain connected to their community.

What You'll Be Doing

Counselling & Support

Provide confidential and effective social work and counselling services to inpatients,



outpatients, families and community members using a social model of health framework.

Discharge Planning & Recovery Support

Support patients transitioning from hospital to home by coordinating recovery plans, linking with services and assisting with specialist appointments and follow-up care.

Advocacy & Care Coordination

Advocate for patients and assist them to access community supports, allied health services, NDIS services, aged care services and other recovery-focused programs.

Community Outreach

Undertake outreach activities, including support within clients' homes where appropriate, to promote recovery, independence and wellbeing.

Multidisciplinary Collaboration

Work closely with mental health, nursing, allied health and community services to support coordinated care and positive outcomes.

What You Bring

- Bachelor of Social Work (desirable)
- Experience in inpatient and/or outpatient social work settings
- Mental health experience and a recovery-oriented approach to care
- Strong counselling, assessment and case management skills
- Excellent communication and stakeholder engagement abilities
- Ability to work independently and within a multidisciplinary team
- Strong advocacy and care coordination experience
- Commitment to person-centred and evidence-based practice
- Current Driver's Licence
- NDIS Worker Screening Check, Working with Children Check and Police Check (or willingness to obtain)
- Rights to work in Australia
