Placed.au is looking for a full-time Disability Services Officer for our client in Seaford, Victoria. This role is responsible for supporting individuals with disabilities, providing community access, and helping them enhance their quality of life and achieve their personal goals.

: -Develop comprehensive and personalised support plans that align with participants’ strengths, preferences, and aspirations, ensuring that goals are realistic, measurable, and achievable. -Regularly review and update support plans in consultation with participants and their support networks, adapting strategies and interventions as needed to meet changing needs and circumstances. -Ensuring that all activities and support plans comply with relevant disability service policies, laws, and ethical guidelines. -Design and facilitate skill-building activities and workshops tailored to participants’ individual needs and interests, focusing on areas such as communication, social skills, personal development, and self-care. -Encourage and support participants to participate in community-based activities and events that align with their interests and goals, fostering social inclusion, community connection, and meaningful engagement. -Coordinate and oversee the development of skill-building programs and activities for participants, ensuring they are tailored to individual needs. Monitor progress, provide guidance, and collaborate with support staff to offer constructive feedback and encouragement that fosters confidence and independence. -Collaborate with allied health professionals, educators, and community-based organisations to access specialised resources and expertise, ensuring that participants receive comprehensive and holistic support. -Work closely with families to optimise a client’s disability support services.



-Assisting individuals in developing skills for greater independence, and providing training on life skills, vocational preparation, and social integration. -Prepare written reports, assessments, and case notes to communicate participant progress, goals, and support needs to relevant stakeholders, ensuring confidentiality, accuracy, and compliance with privacy regulations. -Maintain accurate and up-to-date records of participant interactions, progress, and outcomes, documenting observations, achievements, and challenges in accordance with organisational policies and procedures. -Collaborate with support staff to ensure the delivery of individualised support services. -Foster a supportive and empowering environment where participants feel valued, respected, and encouraged to express their needs and preferences. -Facilitate emotional well-being by ensuring that support plans include appropriate strategies for emotional support. Coordinate with support staff to address participants' concerns, fostering a supportive environment that encourages open communication and reassurance. -Raise awareness and educating the broader community about disability rights, inclusivity, and support services. -Other duties as directed.

Administrative

Functions -Report all incidents and potential hazards promptly, and in accordance with Lifestyle Centred Service’s policies and procedures.



-Ensure time sheets are completed clearly and accurately and submitted to payroll in a timely fashion. -Where applicable, completion and signing of all charts, forms and associated paperwork while undertaking shifts, (eg. signing of pre-drawn medication forms).

Qualifications / Attributes -Must hold a minimum qualification Diploma in Community Services, Disability Support or equivalent, or at least three (3) years of relevant work experience in lieu of the qualification. -Must possess at least a minimum of two (2) years of post-qualification work experience. -Must have strong interpersonal and communication skills, with the ability to build rapport and establish trusting relationships. -Must be able to exhibit empathy, patience, and a genuine passion for individuals with disabilities. -Ability to work independently and as part of a team, with a proactive and flexible approach. -An understanding and commitment to the organisation’s vision and mission statement, and adherence to all company policies and procedures. -Responsible approach to confidentiality and the ability to strictly adhere to the terms and conditions as outlined in the Deed of Confidentiality. -Undertake all relevant training, career development and up-skilling opportunities in accordance with industry and legislative requirements, and to provide support of the highest quality. -Mandatory Disability Sector Conditions of Employment -A cleared NDIS Worker Screening Check. -First Aid Certificate (Course Code: HLTAID011/HLTAID003). -CPR certificate (Course Code: HLTAID009/HLTAID001). -NDIS Worker Orientation “Quality, Safety and You” Certificate, provided by the NDIS Commission. -NDIS Induction Certificate, provided by the NDIS Commission.

Salary Range $90,000 – $105,000 per annum excluding superannuation.
