Out of Hospital – Home Care Worker (Wagga Wagga)

Community Services #1 is seeking a Casual Out of Hospital Home Care Worker to deliver high-quality, person-centred care that supports independence and wellbeing.

About the role:
n
n
- Deliver home care services aligned to Out of Hospital Care goals.
n
- Provide domestic assistance, personal care, support services that assist recovery and independence.
n
- Assist participants during the end of life stages of care.
n
- Apply duty of care and safe work practices in client homes, and identify, document, and escalation triggers promptly.
n
n

About you:
n
n
- Certificate III in Individual Support (Ageing) or equivalent,
relevant experience delivering home care/aged care support (domestic assistance and/or personal care) and 3+ months aged carer experience.
n
- Current First Aid and CPR
n
- Current Driver's License and reliable vehicle with comprehensive insurance.
n
- Able to obtain required pre-employment checks (Police Check and Working with Children Check)
n
- Robust documentation and confidentiality practice.
n
n
