Join our Youth Bloom Transition Services across the Gold Coast and Logan regions, delivering personalised, strengths-based support that empowers young people to achieve their goals, strengthen community connections, and create positive futures.
As a Youth Transition Officer, you will support young people aged 15-21 to build independence, develop practical life skills, and successfully transition into adulthood. Through tailored case management, goal-focused support, and strong community engagement, you will empower young people to access education, employment, housing, and other key services. Working directly with young people in their communities, you will build trusted relationships, provide hands-on guidance, and help them achieve positive and sustainable outcomes.
Responsibilities
- Provide advanced person-centred case management including assessments, transition planning and support for young people experiencing early psychosis, including complex and high-risk presentations.
- Develop and implement tailored support plans that build life skills, resilience, confidence, and community connections.
- Coordinate referrals and collaborate with health, education, employment, housing, and community services to ensure integrated support.
- Advocate for young people and foster strong relationships with key stakeholders to improve access to opportunities and services.
- Maintain accurate case records, monitor outcomes, and contribute to service quality, compliance, and continuous improvement initiatives.

Requirements
- Tertiary qualifications in youth work, community services, or equivalent demonstrated industry experience
- Experience supporting young people in a community and outreach setting
- Current Driver Licence
- Current Blue Card
- Current National Police Check
