Job Description

The Royal Melbourne Hospital is seeking a Diabetes Educator to join the HARP Diabetes Co-Management Service. This part time clinical role (30.40 hours per fortnight) will deliver care across clinic, home-based and telehealth settings for people with unstable diabetes who are at risk of hospital admission.

You will work with consumers, families, GPs and RMH teams, providing diabetes education and self-management support, underpinned by advanced nursing practice and specialised education.
