The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Are you a Registered Nurse who is passionate about supporting children and families during the early years of life? We are seeking dedicated Registered Nurses to join the Child and Family Health Nursing (CFHN) Service. This is a rewarding opportunity to work within a supportive multidisciplinary team, delivering evidence-based care that helps children and families thrive.

CFHN Service forms an integral part of primary health care. The Registered Nurse Child and Family Health aims to improve and promote the optimal health, growth and wellbeing of children aged 0-5 years and their families by providing safe, compassionate care through the assessment, planning,



implementation and evaluation of nursing care in collaboration with families and the multidisciplinary team to achieve outcomes that meet the goals of the child and their family.

Working as part of the multidisciplinary team and utilising a strengths-based family partnership approach, in line with the principles of Primary Health Care, the Registered Nurse will work in collaboration with clients, families and carers who attend the Child and Family Health Nursing Service to provide compassionate, evidence-based, quality client-centred nursing care to achieve identified goals and outcomes. This comprehensive service involves assessment, prevention, early identification and intervention, referral, health promotion, community development and education to clients of the service.

What you will bring to the role

1. Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioners Regulation Authority (AHPRA)
2. Demonstrated recent experience and clinical skill in provision of care to children aged 0-5 years and their families/carers as a registered nurse (including completion of a new graduate RN program or 12 months full time equivalent experience working as a Registered Nurse)
3. Demonstrated capacity to provide effective nursing care that is person centred, family inclusive and culturally sensitive to people accessing health services.
4. Demonstrated ability to organise and prioritise work.
5. Demonstrated effective communications written and verbal skills.
6. Demonstrated commitment to professional development.
7. Ability to work effectively within a multi-disciplinary team.
8. Current driver's licence and willingness to use for work purposes. This may include driving long distances across the LHD.
