Support Coordinator

Aus Support | Toowoomba, QLD Aus Support is looking for an experienced, organised and passionate Support Coordinator to join our growing team.

This is a fantastic opportunity for someone who understands the NDIS, enjoys building genuine relationships with participants and families, and is confident coordinating supports, communicating with stakeholders and helping participants get the most out of their NDIS plans.

About the Role

As a Support Coordinator, you will work closely with participants to help them understand and implement their NDIS plans, connect with appropriate providers and community supports, and build their confidence and capacity to navigate the NDIS. You will manage your own participant caseload while working alongside the Aus Support management and operations team.

Your key responsibilities will include:

- Supporting participants to understand and implement their NDIS plans
- Building solid relationships with participants, families and stakeholders
- Connecting participants with suitable providers, allied health professionals and community services
- Coordinating and monitoring participant supports
- Monitoring NDIS plan utilisation and Support Coordination budgets
- Identifying gaps in services and assisting participants to find appropriate solutions
- Liaising with the NDIA, plan managers, providers and other stakeholders
- Supporting participants with plan reassessments and changes in circumstances
- Attending stakeholder and care-team meetings
- Preparing progress reports and other NDIS documentation
- Maintaining accurate and timely case notes
- Identifying and escalating participant risks or safeguarding concerns
- Supporting participant choice, control and independence
- Maintaining professional boundaries and confidentiality

About You

We are looking for someone who is approachable,



reliable and confident working independently while still being part of a supportive team.

You will ideally have

- Previous experience in NDIS Support Coordination, case management or the disability/community services sector
- A solid understanding of the NDIS and participant plans
- Excellent communication and relationship-building skills
- Strong organisation and time-management skills
- Confidence communicating with participants, families, providers and allied health professionals
- Strong written documentation and report-writing skills
- Ability to manage competing priorities and follow through on outstanding actions
- A participant-focused approach with a strong understanding of choice and control
- Confidence using computer systems and participant management software

Essential Requirements

The successful applicant will be required to hold or obtain relevant employment screening and compliance requirements, which may include:
- NDIS Worker Screening Clearance
- Current Australian Driver Licence
- Reliable vehicle
- NDIS Worker Orientation Module
- Working with Children Check/Blue Card where applicable
- Relevant qualifications and/or demonstrated industry experience
