Support at Home Care Partner/Clinical Care Partner

Permanent Part Time

0.6 FTE - 48hrs Per Fortnight The Support at Home (S@H) Care Partner/Clinical Care Partner Coordinates and manages tailored support services to help older people live safely and independently at home. Working within a person-centred, rights-based approach, you will partner with participants, families, and multidisciplinary teams to plan, monitor, and review care in line with individual goals, wellbeing, and reablement outcomes.

You will support informed choice by helping consumers select preferred service providers and manage program funding. The role also includes promoting REDHS services and engaging eligible clients.

Overseeing services across all streams—including Assistive Technology and Home Modifications (AT-HM)—you will collaborate with clinically qualified colleagues when complex or advanced care needs arise.
