About the role

Empower people to access the care they deserve. Working within a multidisciplinary, human rights-focused team, you’ll help vulnerable older people connect with aged care, housing and community supports to improve wellbeing, independence and quality of life.

Your duties include:

- Conduct assertive outreach to identify and engage vulnerable older people who may benefit from the Care Finder program
- Assist clients to register with My Aged Care and access government funded MAC services
- Develop and implement goal-directed support plans that reflect client priorities and needs
- Build trusting and respectful relationships with clients through a trauma informed, strengths based approach

Our ideal candidate:

- Relevant tertiary qualification in Social Work, Community Services, Human Services or a related discipline, or equivalent experience
- Demonstrated experience working with vulnerable people with multiple and complex needs
- Knowledge of the Social Determinants of Health, trauma informed practice and strength-based approaches to supporting vulnerable individuals
- Demonstrated understanding of case management principles, including intake, assessment, care planning, advocacy, referral and service coordination

All appointments are made subject to a satisfactory Nationally Coordinated Criminal History Check (NCCHC), a Working with Children Check (WWCC), complete a Pre-Employment Declaration Form, evidence of legal rights to work in Australia, and evidence of vaccination.

Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
