About the Opportunity

We are looking for a dynamic, passionate, and experienced Social Worker to work within our fantastic team of highly experienced doctors, registered nurses and administrative team members at Ochre Medical Centre Bruce.

Location: Ochre Medical Centre Bruce ACT

Core Responsibilities:

- Provide psychosocial assessment, brief counselling, crisis intervention, and short-term casework for patients experiencing social, emotional, family, mental health, or situational stressors

- Support patients experiencing family violence, housing instability, financial stress, chronic illness adjustment, grief, trauma, and caregiving pressures.

- Work collaboratively with GPs, nurses, practice staff, and allied health professionals to support holistic, team-based care

- Support patients to navigate complex health and social service systems





- Facilitate warm referrals and coordinated care with external services (e.g. community services, housing, mental health, family violence services, NDIS, aged care, legal and financial supports).

Essential Criteria:

- Bachelor or Master of Social Work

- Minimum 5 years' post-qualification experience across health, community, or primary care settings

- Experience in general practice, primary care, or integrated care settings

- Ability to work autonomously and collaboratively in a fast-paced general practice setting
