The Homeless Health Outreach Team (HHOT) provides comprehensive mental health assessment, care co-ordination and clinical intervention for homeless persons in the community who are experiencing mental illness. The service aims to minimise the health problems of people experiencing homelessness and contribute to the prevention and reduction of homelessness in Queensland through cross-sectoral collaboration, to facilitate access to the most appropriate supports. Services are provided to individuals who are: homeless, sleeping rough, in emergency accommodation, in specialist homelessness services or couch surfing.

HHOT has three service delivery components:

- assertive outreach
- comprehensive assessments and holistic care coordination
- collaborative service response.

The service is recovery oriented, person centred, strengths based, collaborative and empowering; and operates on the premise that consumers can and do recover from mental illness.

Key Responsibilities include:

- Provide advanced mental health nursing knowledge, expertise and skill in the provision of comprehensive assessment, care and treatment for people identified with mental health diagnoses and provide care management services including therapeutic engagement, education, clinical assessment, risk assessment, and delivery of evidence based therapeutic interventions.
- This position requires you to adhere to National Safety and Quality Health Service (NSQHS)



Standards and the Sunshine Coast Hospital and Health Service Safe Care Framework, which has been developed in collaboration with staff, consumers and key stakeholders to enable our vision of health and wellbeing through person-centred care.
- Fulfil the responsibilities of this role in accordance with Sunshine Coast Hospital and Health Service Values.

Remuneration value up to $92 354 p.a, comprising salary rates $57.76 - $61.85 p.h., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)). (Part time position, 50.16 hrs p.f.). Applications will remain current for 12 months.
