Contract inclusions

- Overview of Responsibilities

- Provide holistic and culturally safe care to Aboriginal and Torres Strait Islander peoples, families and communities
- Act as a liaison between patients, families, communities and health care providers, supporting continuity of care and access to services
- Support community-led health and wellbeing initiatives and contribute to local health promotion activities
- Share health information in a culturally appropriate way that empowers people to make informed decisions about their health and wellbeing
- Carry out clinical assessments,



tests and procedures within your scope of practice while working collaboratively with multidisciplinary teams

Must have

- AHPRA registration as an Aboriginal and Torres Strait Islander Health Practitioner
- Certificate IV or higher in Aboriginal and Torres Strait Islander Primary Health Care (Practice).
- Experience working as an Aboriginal and Torres Strait Islander Health Practitioner in a community, primary health care or related setting
- Demonstrated ability to build respectful relationships with Aboriginal and Torres Strait Islander peoples, families, communities and stakeholders
