- Ensure the team complies with the Disability Services Act Standards, NDIS framework and other relevant legislation.
- Apply person-centred approach and solution-focused, and strengths-based approach
- Coordination and development of clients’ care plans in liaison with families and other stakeholders.
- Maintaining of participants records and providing updates on progress in weekly, monthly or quarterly reports.
- Ensuring high-quality service delivery that aligns with individuals' needs and goals.
- Ensure a high level of participants satisfaction measured through regular scheduled feedback and review processes.
- Coordinate and oversee rostering and timesheet requirements according to client support needs.
- Assisting young people to solve social, emotional and financial problems.
- Assist clients with daily living activities, such as personal care, meal preparation, household tasks and maintaining routines.
- Support clients with appointments, transport and community access.
- liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
- Development of professional relationships with the participants, their families and other stakeholders and engagement and regular stakeholder meetings.
- Provision of accurate and timely case notes, incident reports and house reporting.
- Assessing clients’ needs and planning, developing and implementing educational, training and support progress.
- Coordination and management of day-to-day operations in our Supported Independent Living program.
- Interviewing clients and assessing the nature and extent of difficulties
- Contributing to the development and implementation of service improvements.
- Conducting regular visits to clients home and community locations.
- Assessing community need and resources for health, welfare, housing, employment, training and support programs.
- Provide pre-planning support for new clients and families/carer transitioning to the agencies that can provide additional help.
- Facilitating communication and collaboration between different teams and departments.
- Developing and reviewing the client support plan
- Continue as the point of contact, identifying clients’ needs and ensuring they are communicated with regularly, and this is documented prior to formal admission to the service.
- Monitoring and auditing clients' files to ensure accuracy and compliance.
- Identify and report risks, abuse, neglect or incidents in accordance with legislation and workplace policies.
- Develop positive, respectful relationships with clients and promote their independence, choice and dignity
- Monitor and report changes in clients’ health, behaviour, wellbeing or support needs.
- Encourage development of life skills, including communication, decision-making, budgeting and independent living skills.
- Respond appropriately to challenging behaviours or complex support needs, using approved strategies.
- Work collaboratively with families, carers, support coordinators, nurses, allied health professionals and other service providers.
- Maintain confidentiality, privacy, safeguarding and qualified boundaries.
- Participate in team meeting, training, supervision and ongoing professional development.
The successful applicant will demonstrate:
· Certificate III, IV or degree in Disability, Diploma of Community Service or other relevant qualification.
· Knowledge of the NDIS Quality and Safeguarding Framework.
· NDIS knowledge and awareness.
· Extensive knowledge regarding relevant legislation in the disability sector.
· Experience working in the Disability and Community setting with an accommodation facility supporting clients with high/complex needs.
· Experience supporting people with high support needs and personal care.
· Experienced supporting people with disability and behaviours of concern utilizing an empathetic and person-focused approach.
· Demonstrated capacity to manage crisis and critical issues and deal with complex and challenging situation.
· Demonstrated capability in risk identification, management, documentation and escalation.
· Strong Coordination, problem-solving, and leadership skills.
· The ability to contribute positively within a team.
· Experience developing NDIS plans, assessments or equivalent participant-facing documentation.
· Excellent communication skills, including computer skills, internet, email, Microsoft Word, Excel, and PowerPoint.
· Ability to use computers, tablets and smartphones
· A positive mindset, proactive can-do attitude, solution-focused.
Requirement:
· NDIS Worker Screening Check and NDIS Module
· Valid First Aid Certificate with current CPR
· Current Working With Children Check (WWCC)
· A current unrestricted NSW Driver License with comprehensive insurance
· Current Police Check
· 100 Points valid identification
