We're excited to welcome a Social Worker Senior to our Social Work Acute Care Service Team based at Sunshine Coast University Hospital, Birtinya.

The Geriatric Evaluation and Management and Hospital in the Home (GEM / HITH) Team provides high-quality social work services to often older patients following discharge from hospital, through a home-visiting service. Experience in therapeutic interventions relating to adjustment to a new diagnosis, anxiety management, dementia support, carer support, discharge planning, and complex grief and loss counselling would therefore be highly desirable.
As a highly functioning multidisciplinary team member, deliver high level Social Work clinical services within the home, to stakeholders/customers of the Sunshine Coast Hospital and Health Service and Queensland Health (SCHHS).
Key Responsibilities include:
- Deliver independent high level social work clinical practice with infrequent need for direct clinical practice supervision. Exercise professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence.
- Provide high level clinical advice, education and influence towards a common patient-centred vision and direction to professional operational teams and relevant stakeholders regarding service delivery.
- Provide leadership oversight and clinical practice supervision to less experienced social work practitioners and students as delegated.
- Adhere to relevant expectations outlined in the SCH and Allied Health Governance Framework documents and Business Rules to ensure that best patient outcomes are achieved in a transparent, ethical and accountable manner.
Remuneration based pro rata on equivalent full time value up to $157 359 p.a., comprising salary rates: $64.64 - $69.55 p.h., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (HP4) (Fixed term temporary part time position available until June 2028, 30.4 hrs p.f.).
