Carers NSW is the peak organisation for carers in NSW. We are a not-for-profit organisation connecting and empowering carers across NSW and improving their quality of life through activities, programs, training and policy advice. We advocate for carers of all ages and circumstances.

The Mobile Intake & Assessment Outreach Worker is responsible for engaging with carers across the New England region through outreach activities, intake, needs assessment and support planning processes.

This role delivers person-centred assessment and support planning services, facilitates referrals to Carer Gateway and other relevant supports and developments strong partnerships with community organisations and service providers to increase awareness of and access to Carer Gateway services.

This is a part-time contract position, offering 4 days per week (30 hrs), until 30 June 2027.

Key responsibilities include:

- Travel throughout New England region to engage with carers and act as a primary point of contact for intake and assessment services

- Conduct carer registration, intake, needs assessment and support planning processes using approved assessment tools, including Carer Star™.

- Assess eligibility for available services and identify appropriate support pathways based on individual carer needs, including referral to Carer Gateway and Carers NSW services and other community-based supports

- Apply trauma-informed, strengths-based and person-centred approaches when engaging with carers

- Obtain consent from carers to make referrals to access supports and ensure relevant documentation is completed to facilitate the carer support planning process





- Establish and maintain strong relationships with health, community and social service organisations to increase referrals and service access to carers

- Travel to regional, rural and remote communities to strengthen local service networks and community connections

- Promote awareness of the Carer Gateway services through presentations, networking activities, community events and information sessions

- Collaborate with service providers to facilitate seamless referral pathways and coordinated support for carers

To be successful in the role, you will ideally possess:

- Tertiary qualification in a relevant discipline and/or extensive experience

- Experience and/or strong understanding of the intake, assessment and referral processes as part of a support and planning service

- Experience developing and maintaining effective partnerships with community organisations and service providers

- High level of interpersonal, communication and stakeholder engagement skills

- Demonstrated capacity to provide high-quality support, information, practical assistance to support carers

- Sound knowledge of community care, health and support systems relevant to carers

- Strong organisational skills and computer skills, including use of CRM systems.





- Solid understanding of Mandatory Reporting, risk identification, confidentiality and privacy requirements

- Current NSW drivers licence and willingness to undertake regular travel across the New England region
