About the role

Immediate Care is seeking a reliable, respectful and experienced Disability Support Worker to provide individualised support to an NDIS participant living in Ararat. The participant is an adult with an above-knee amputation who requires assistance with daily living, mobility and community participation. We are looking for a worker who can build a positive and skilled working relationship while supporting the participant to maintain independence, choice and control.

Key responsibilities
Assistance with daily living activities and routines
Mobility support and assistance associated with physical disability
Supporting safe access within the home and community
Community access, appointments, shopping and social activities
Assistance with household tasks where required
Encouraging independence and supporting the participant to work towards their NDIS goals
Providing respectful, person-centred support
Maintaining appropriate progress notes and documentation




Communicating with the participant and relevant members of their support team
Following the participant's support plan, risk assessments and Immediate Care policies and procedures

About you
Respectful, patient and dependable
Comfortable supporting a person with a physical disability and mobility needs
Inclusive and respectful of people from diverse backgrounds, identities and lifestyles
Able to maintain skilled boundaries while developing a genuine and positive working relationship
Confident supporting community participation and independence
A positive communicator who listens to and respects the participant's choices and preferences
NDIS Worker Screening Check
Working with Children Check, where applicable
Current First Aid and CPR
Current Australian driver's licence
