About the Opportunity
Note: This is a part time (3 days per week) fixed term position till 30 June ****
As a ReConnect Youth & Family Worker, you will provide youth-centred, family-focused case management and practical support to vulnerable young people and their families.
Working within a strengths-based and trauma-informed framework, you will:
Deliver case management and support services to young people and families
Conduct assessments and develop individualised case plans
Facilitate group programs that build life skills, social connection and wellbeing
Use assertive outreach strategies to engage young people experiencing complex challenges
Work collaboratively with government, community and family support services
Support community capacity-building initiatives and awareness activities
Maintain high-quality client records,
data and reports
To be successful in this role, you will bring:
Qualifications in Human Services, Social Sciences or a related field
Experience working with vulnerable or high-risk young people and families
Knowledge of youth, family support, homelessness and child protection systems
Experience applying trauma-informed, strengths-based and youth-focused practice frameworks
Strong assessment, case management and engagement skills
Experience facilitating groups and workshops for young people
Excellent communication, relationship-building and stakeholder engagement skills
