Western Health in Melbourne's west seeks an experienced Emergency Department Care Coordinator for a 12-month fixed-term, full time role across Sunshine, Footscray and Williamstown hospitals. You will coordinate care for patients with complex needs and work with a transdisciplinary team to reduce avoidable admissions and delays.
