About the role Access Disability Support Services is seeking an experienced Disability Support Worker to join our Perth team.
You will provide practical, respectful and person-centred support to people with disability and older clients, helping them live safely, independently and actively within their homes and local community.
Key responsibilities Assist clients with personal care, mobility, meal preparation, household routines and daily living activities in accordance with their individual support plans.
Encourage clients to build confidence, maintain life skills, access community activities and develop social connections.
Provide transport and support clients to attend appointments, programs, shopping and other community-based activities where required.
Monitor each client's wellbeing and promptly report changes, incidents, hazards or safeguarding concerns to the relevant supervisor.
Follow individual plans, risk assessments, behaviour support strategies and directions from the client's care team.
Maintain respectful communication with clients, families, coordinators and other service providers while protecting privacy, choice and dignity.
Complete clear progress notes, incident reports and other required service documentation.
Follow all workplace health and safety, infection control, NDIS,



privacy and duty of care requirements.
About you To be considered, you will have: Certificate III or Certificate IV in Individual Support, Disability, Ageing, Community Services or a related qualification.
At least two years of relevant full-time experience providing care and support to older people or people with disability.
Practical experience assisting with personal care, community access, daily living support and implementation of individual support plans.
A sound understanding of client rights, professional boundaries, safeguarding and duty of care.
Solid communication, documentation and time-management skills.
The ability to work independently and as part of a supportive care team.
Mandatory requirements NDIS Worker Screening Check National Police Check Working with Children Check, where required Current First Aid and CPR certification, or willingness to obtain it Valid driver licence and access to a reliable vehicle Two recent professional references Experience supporting NDIS participants, people with complex needs, dementia or mobility limitations will be highly regarded.
