Where you'll be working
Join the Homeless Persons Mental Health Team at Jeffrey House, Parramatta, part of Western Sydney Local Health District's diverse and innovative community mental health services. You'll work alongside experienced clinicians and partner with health, government and community services to improve access to care for people experiencing homelessness.

- Work within a specialist mental health service supporting people who are homeless or at risk of homelessness.

- Deliver care across acute, community and outreach settings, connecting consumers with the services they need.

- Collaborate with GPs, hospitals, police, government and non-government organisations and community agencies.

- Be part of a culture grounded in collaboration, respect, empowerment, recovery and quality improvement.

- Support people across Parramatta, Auburn, Blacktown and the wider Western Sydney community.

What you'll be doing
As a Mental Health Clinician – Registered Nurse, you'll bring your clinical expertise and person-centred approach to a role where every interaction can help someone move towards greater stability, health and wellbeing.

- Provide acute clinical mental health care, including triage, comprehensive assessment, crisis intervention and service coordination.

- Develop individualised care plans with consumers, carers and other providers,



incorporating prevention and early intervention strategies.

- Build therapeutic relationships and connect consumers with GPs, hospitals, NGOs and other health and community services.

- Support medication management, clinical reviews, risk management, clinical documentation and outcome evaluation.

- Contribute to multidisciplinary teamwork, professional education, quality improvement, service development, evaluation and health promotion.

Essential Requirements:

Qualifications: Relevant Tertiary qualification as a Registered Nurse
Registration: Full Registration with AHPRA as a Registered Nurse

Selection Criteria:

1. Experience and clinical skills in community mental health triage, comprehensive assessments, including risk and safety planning as well as experience in provision of collaborative care and discharge planning

2.



Understanding of Mental Health/ Psychiatry service/ crisis work and ability to work with in a demanding crisis/acute management service.

3. Demonstrated ability to collaborate with consumers/carers/families and has experience working with families who are coping with Mental Health Illness.

4. Highly developed interpersonal communications skills, effective verbal, written and negotiation skills and intermediate computer skills.

5. Experience and knowledge in acute mental health intervention, early intervention, and CALD issues.

6. Understanding of the recovery philosophy and trauma informed care in Mental Health with the ability to implement these in clinical practice.

7. Understanding of NSW Mental Health Act (2007).

8. Current NSW Drivers Licence (P2 acceptable) and willing to use it for work purpose.
