Our Hospital in the Home (HITH) service enables patients who would otherwise require hospital admission to safely receive acute-equivalent treatment in their homes, workplaces, residential aged care facilities, or through virtual care pathways.

About the Opportunity
We're seeking experienced Clinical Nurses to join our growing Hospital in the Home (HITH) team based at St Vincent's Private Hospital Sydney.
As a HITH Clinical Nurse, you'll deliver high-quality, evidence-based care outside traditional hospital walls, supporting patients with acute and complex healthcare needs in their preferred environment.
Working closely with Clinical Leads, Visiting Medical Officers,
allied health professionals and community providers, you'll play a critical role in ensuring patients receive safe, coordinated and compassionate care while maintaining hospital-level clinical standards.
Key Responsibilities
- Conduct comprehensive patient assessments and clinical reviews.
- Develop, implement and evaluate individualised care plans.
- Deliver advanced nursing interventions, including IV therapies and complex wound management.
- Monitor patients with acute and complex clinical needs and escalate care when appropriate.
- Coordinate referrals, admissions, transitions of care and discharge planning.
- Collaborate with multidisciplinary teams to ensure seamless care delivery.
- Educate and support patients, families and carers.
- Participate in service development, quality improvement and clinical governance activities.
- Mentor and support new and junior team members.
About You
You're an experienced Registered Nurse who enjoys building meaningful patient relationships and thrives in an environment that combines autonomy, innovation and clinical excellence.
You are passionate about delivering high-quality care in the community and are excited by the opportunity to help shape the future of connected healthcare.
You Will Bring
- Current registration as a Registered Nurse with AHPRA.
- Minimum three (3) years' post-graduate nursing experience.
- Experience in community nursing and/or acute medical, surgical, emergency, ICU or critical care environments.
- Demonstrated experience managing patients with complex clinical needs and multiple co-morbidities.
- Strong clinical assessment, care planning and problem-solving skills.
- Excellent communication and stakeholder engagement capabilities.
- Ability to work independently while contributing effectively within a multidisciplinary team.
- Strong digital literacy and confidence using virtual care technologies.
- Current unrestricted NSW Driver's Licence.
- Full and unrestricted working rights in Australia.
Highly Regarded
- Hospital in the Home (HITH), community nursing, virtual care or care-at-home experience.
- Postgraduate qualifications in nursing or related specialty areas.
- Experience contributing to service innovation, quality improvement or clinical projects.
