About The Role

Join Limestone Coast Local Health Network as a Community Care Coordinator, supporting the delivery of community-based aged care, disability and home support services.

Working as part of a multidisciplinary team, you will coordinate, implement and evaluate services that enable consumers to remain living independently in their own homes. With a strong focus on Consumer Directed Care, you will work with consumers and carers to identify goals, coordinate appropriate supports and ensure services are responsive to individual needs.

You will also oversee service provision and staff allocation, support quality and compliance requirements, and build effective relationships with health professionals, government agencies and community service providers.

About You

You are an organised and capable coordinator with experience in community, aged care, disability or health services. You can balance consumer needs with operational requirements, build solid working relationships and confidently coordinate staff and services. You bring sound judgement, initiative and a genuine commitment to consumer choice, independence and quality care.

To succeed in the Community Care Coordinator role, you will:

- Have demonstrated experience and proficiency in a managerial or supervisory role, including the supervision of staff.
- Bring experience in aged care, palliative care, rehabilitation, disability or community health services and working within a multidisciplinary team.
- Demonstrate strong organisational, problem-solving and decision-making skills, including the ability to manage priorities and respond effectively to complex situations.
- Have the ability to liaise effectively with health professionals, human service agencies, consumers, carers and community groups.
- Demonstrate an understanding of Consumer Directed Care,



community-based service delivery, quality improvement and the needs of frail aged and people living with disability.

About Home Support Services

LCLHN Home Support Services provide community-based aged care, disability and home support services designed to help consumers maintain greater control over their lives and remain living independently in their own homes.

Using a Consumer Directed Care approach, consumers are supported to make choices about the services they receive, identify their individual goals and determine how involved they wish to be in managing their care arrangements. The Community Care Coordinator works across multidisciplinary services and programs to coordinate supports that are flexible, responsive and focused on individual consumer needs.

Conditions of Employment

- A current Nationally Coordinated Criminal History Check (NCCHC) is required for this position.
- Relevant screening requirements associated with working with vulnerable people and aged care services apply.
- Immunisation Risk for this position is Category A – minimal patient contact.
- A current driver’s licence and willingness to drive is required.
