Job Description
Mounties Group in NSW is seeking a dedicated Care Partner to join our Mount Pritchard site on a full time basis. You will coordinate client care and services, helping clients stay independent at home and in the community, while supervising a team of carers to ensure quality outcomes.

You will develop care plans, manage budgets, assist with My Aged Care access, visit clients, and engage with families and providers to ensure seamless care delivery and adherence to standards.
