Job Description
healthAbility invites applications for the Health Navigator – Pathway to Valuable Health role, a fixed term, part time (0.6 FTE) contract in Melbourne.
The Health Navigator supports children and young people in Out of Home Care with trauma-informed assessments and care coordination.
You will work with multidisciplinary teams to connect clients with health, wellbeing and specialist services, advocate for access to healthcare, and maintain accurate records in line with program requirements.
