Sydney Local Health District invites applications for roles within the Concord Repatriation General Hospital.
You will participate in a multidisciplinary Aged Care Assessment Service, triaging referrals via the My Aged Care Assessor portal and conducting comprehensive aged care assessments to develop care plans for frail older people.
The ideal candidate will have robust communication skills, the ability to triage referrals, and hold a current NSW Driver's Licence.
