What you will own

· Client relationships. Be the primary point of contact for clients and families, from referral and assessment through to ongoing reviews.

· Care planning. Conduct comprehensive in-home assessments and build personalised plans around each client's strengths, priorities, and goals.

· Budgets and agreements. Develop individual budgets, manage service agreements, and monitor package utilisation in line with Support at Home requirements.

· Service coordination. Partner with Scheduling, care workers, allied health professionals, and other stakeholders to deliver seamless support.

· Quality and compliance. Maintain accurate records and ensure services meet the Aged Care Quality Standards and regulatory requirements.

· Better ways of working. Spot opportunities to strengthen the client experience, improve service delivery and support sustainable growth.

You will thrive here if you

· Lead with people. You are warm, perceptive, and genuinely person-centred, with the confidence to hold clear and respectful conversations.

· Know community care. You bring at least one year of relevant case management experience in home-based aged care or community services; deeper experience is highly regarded.

· Understand the detail. You can manage budgets, priorities, documentation, and compliance without losing sight of the person behind the plan.

· Stay calm and find a way forward.



You are organised, digitally capable and comfortable making decisions in a changing environment.

· Bring relevant training. A diploma or tertiary qualification in aged care, community services, disability, social work, nursing, allied health, or a related field is highly regarded. A nursing background is desirable.

· Can travel locally. You hold a valid Victorian Driver Licence.

· NDIS Worker Screening Check. You have or are willing to obtain the required NDISWSC clearance.
