Join the Ambulatory Care team at Griffith Base Hospital as a Clinical Nurse Specialist Grade 2 and play a key role in delivering safe, coordinated and innovative care for people experiencing non-emergent urgent health needs.

This is an exciting opportunity to work across Hospital in the Home (HITH), Aged Care Outreach Service (ACOS), and Procedures, Investigations and Infusions (PIXI), while partnering with multidisciplinary teams across the region. You will also work autonomously while collaborating closely with community services, Residential Aged Care Homes and external providers to ensure patients receive the right care, in the right place, at the right time.

About the Opportunity

As the CNS2 Ambulatory Care, you will provide advanced clinical assessment, leadership and support across a diverse range of Hospital, ambulatory and community-based care settings.

In this role you will:

- Provide timely and comprehensive clinical assessments for patients presenting to HITH, PIXI and ACOS services.

• Undertake clinical and risk assessments and develop safe, evidence-based management and safety plans.
- Coordinate patient care, discharge planning, referrals and community follow-up to promote seamless transitions of care.
- Support admission and patient flow processes, including liaison with inpatient services and other MLHD facilities.
- Provide clinical leadership, specialist advice, mentoring and support to nurses and other health professionals.




- Work collaboratively with HITH, ACOS, PIXI, RACHs and community services to support ongoing monitoring, review and discharge planning.
- Assist with the allocation of workloads and management of complex clinical presentations.
- Identify opportunities to improve models of care, clinical processes and service delivery.
- Act as a professional role model and advocate for patients, families and carers.
- Promote evidence-based practice, education, learning and continuous improvement across the service.

About You

Our ideal candidate will demonstrate:

- Current registration with AHPRA as a Registered Nurse (Division 1).
- Relevant postgraduate qualifications and at least three years' full-time equivalent clinical experience.
- Advanced clinical assessment, clinical decision-making and patient management skills.
- Demonstrated experience managing complex presentations and identifying and responding to clinical risk.
- Excellent verbal, written and interpersonal communication skills.
- The ability to work autonomously while contributing effectively within a multidisciplinary team.
- Demonstrated clinical leadership and a commitment to developing nursing practice and service delivery.
- Robust digital and information technology skills and the ability to use technology to support contemporary clinical care.
- A commitment to evidence-based practice, innovation and improving patient outcomes.
