Duties and Responsibilities
Family Support and Case Work
- Assess the needs and circumstances of families referred to the service, including family functioning,
parenting capacity, safety, health, housing and financial wellbeing, and identify the supports required.
- Develop, implement and review individual and family support plans with measurable goals, in
collaboration with the family, the Case Manager and the multidisciplinary team.
Deliver structured in-home and community-based family support, including parenting skill development, household routine and budgeting support, engagement with health and education services, and connection to community resources.
- Monitor and report on each family’s progress against plan goals and adjust supports as
circumstances change.
- Provide practical assistance and emotional support to parents, carers, children and young people,
applying strengths-based, child-centred and trauma-informed practice.Disability and NDIS Support
- Support NDIS participants and their families to work towards the goals set out in their NDIS plans
across daily living, social and community participation, and capacity-building supports.
- Implement behaviour support and therapeutic strategies developed by clinicians, including in
supported independent living and short-term accommodation (respite) settings.
- Assist participants and families to understand and make effective use of their plan funding, and
liaise with Support Coordinators, allied health providers and community nursing staff.
Referral, Advocacy and Stakeholder Liaison
- Assess available community resources and refer and connect families to housing, health, mental
health, legal, education, financial counselling and specialist domestic and family violence services.
- Advocate on behalf of families with government departments, schools, health services and
community agencies.
- Build and maintain effective working relationships with referring agencies and external service
providers.
Documentation and Compliance
- Maintain accurate, timely and objective case notes, progress reports and incident reports in the
organisation’s client management system, to the standard required by funding bodies.
- Prepare written reports and progress updates for funding bodies, referring agencies and, where
required, statutory bodies.
- Work within the NDIS Practice Standards, the NDIS Code of Conduct, organisational policies and
procedures, and all applicable privacy and record-keeping obligations.
Skills / Experience / Specific Qualifications Required:
Essential – Qualifications
- A completed AQF Diploma or higher in Community Services, Human Services, Social Work, Youth
Work, Psychology, Behavioural Science or a closely related discipline (for example, CHC52021
Diploma of Community Services or an equivalent qualification).
- Applicants who do not hold a formal qualification will be considered where they can demonstrate at
least three years of directly relevant, full-time equivalent experience in family support or a closely related field.
Essential – Experience
- A minimum of 12 months’ recent, paid experience delivering family support, child and family
intervention, out-of-home or residential care, or NDIS-funded individual and family supports.
- Demonstrated experience working with families experiencing complex and co-occurring needs –
including disability, mental illness, domestic and family violence, substance use, financial hardship or statutory child protection involvement.
- Demonstrated experience conducting needs assessments and developing, implementing and
reviewing goal-directed support plans.
- Demonstrated experience maintaining case notes, progress reports and incident reports to the
standard required by a government or NDIS funding body.
Essential – Knowledge and Technical Competencies
- Working knowledge of strengths-based, person-centred, child-centred and trauma-informed practice
frameworks, and the ability to apply them in day-to-day case work.
- Sound working knowledge of the Queensland child protection framework, including mandatory
reporting obligations under the Child Protection Act 1999 (Qld).
- Sound working knowledge of the NDIS – participant plans and goals, core and capacity-building
support categories, the NDIS Practice Standards, the NDIS Code of Conduct, and reportable incident obligations.
- Understanding of positive behaviour support principles and awareness of regulated restrictive
practices and the obligations attaching to them.
- Demonstrated risk assessment, safety planning, de-escalation and crisis
Essential – Mandatory Clearances and Licences (held, or obtainable before commencement)
- Queensland Working with Children Check (Blue Card).
- NDIS Worker Screening Clearance.
- National Police Check.
- Current First Aid and CPR certificates.
- NDIS Worker Orientation Module – “Quality, Safety and You” – certificate of completion.
- Current Queensland open driver’s licence, together with a reliable, roadworthy vehicle and
comprehensive motor vehicle insurance. The role requires daily travel between client homes and service sites across the Logan, Beaudesert and Ipswich regions.
- Valid rights to work in Australia.
Desirable
- A Bachelor or Master level qualification in Social Work or Human Services, and eligibility for
membership of the Australian Association of Social Workers or the Australian Counselling
Association.
- Experience in supported independent living or short-term accommodation (respite) settings.
- Experience delivering services under Queensland Government funded in-home family support or
- child and family intervention programs.
- Ability to converse in a second language relevant to the service’s client base – for example Spanish,
- Mandarin, Hindi, Punjabi or Nepali.
- Experience with, or interest in, animal-assisted or equine-assisted therapeutic programs.
- (KEY COMPETANCIES, MUST HAVES)
- Type of Person / Background Suitable:
- A community services, social work or human services practitioner who is comfortable working
- independently in families’ homes and in the community, rather than from a desk.
- Resilient and composed under pressure, able to hold clear professional boundaries while working
- with families in crisis.
- A disciplined documenter – someone who understands that case notes and incident reports are the
- evidence base for the service and for the families it supports.
- A collaborative team member who actively uses supervision and reflective practice.
- Backgrounds in child protection, out-of-home care, domestic and family violence, disability support
- or community mental health are all directly relevant.
- Experience working with culturally and linguistically diverse communities, Aboriginal and Torres
- Strait Islander families, and people with disability is highly regarded.

- Employer to confirm which items are provided by the organisation and to name the client management
- system used.
- Presentation / Dress Code etc:
- Neat, practical clothing suitable for in-home and community work, including enclosed shoes.
- Smart casual attire for stakeholder meetings, case conferences and attendance at external
- agencies.
- Personal protective equipment provided by the organisation where required.
Other Info:
- Reports to: Director/CEO
- Regular travel across the Logan, Beaudesert and Ipswich regions is a core requirement of the role.
- Some evening, weekend and sleepover shifts are required on a rostered basis.
- The organisation is a registered NDIS provider and a child-safe organisation; all staff work within the
NDIS Code of Conduct and the organisation’s child-safe policies.
Pay: $80,000.00 – $82,000.00 per year
