You will deliver high-quality home care to elderly clients, work with the community nurse consultant, and support gerontology, palliative care, dementia, and mental health initiatives.

Key duties include clinical assessments, coordinating appointments, responding to medical and emergency needs, and discharge planning from hospital to home.
