Springwood Hospital was opened in 1976 as a community-based hospital in the Blue Mountains. Springwood community remains very active in supporting and raising funds for the hospital who provides a range of allied health, rehabilitation, aged care and palliative care services.

We are looking for a dedicated and supportive Social Worker who is committed to improving the health and wellbeing of patients, families and carers across the Blue Mountains and Springwood community.

As a Social Worker, you will play a vital role in supporting patients through often complex health and life circumstances. Working within the hospital setting, you will assess and respond to the psychosocial needs of patients while considering the broader factors that influence their health and wellbeing. You will partner closely with multidisciplinary teams to deliver high-quality, patient-centred care and contribute to positive outcomes throughout the patient journey.

In this rewarding role, you will provide a range of social work interventions including crisis intervention, psychosocial assessments, counselling, advocacy, case management, therapeutic support, grief and loss counselling, and complex discharge planning.

What you will bring to the role

1. Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers.
2. Demonstrated clinical experience or understanding of the needs of people accessing the health care system and the psycho-social impacts this may have. Preferred experience in working in an acute hospital setting with knowledge in the areas of crisis counselling, grief and loss, end of life concerns, carer stress, conflict resolution, trauma informed care and identifying and working with complex family dynamic’s
3. Demonstrated ability to complete psychosocial assessments and utilisation of social work theories and practice standards to formulate and implement appropriate discharge and care plans in a short time frame for patients in an inpatient setting.
4. Understanding and/or knowledge in the areas of child protection, domestic and family violence, NDIS, NCAT Guardianship Division external service providers and community services.
5. Demonstrated understanding of with working with people of First Nation background, LGTQI and Culturally and Linguistically Diverse patient groups.
6. Demonstrated ability to work as part of a multi-disciplinary team
7. Well-developed time management skills with a high level of written and oral communication skills
8. Current Drivers Licence valid within Australia and willingness to use for work purpose travel.
