Job Description
Barwon Health's Youth Continuing Care Service seeks a role within Youth CCS to support young people aged 12–25 with moderate to severe mental health and AOD challenges. This position works with YMHWBS to bridge primary and specialist care, delivering long-term treatment in a rotating 7-day roster across sites.
