Job Description

Northern Health in Melbourne's rapidly growing northern suburbs seeks a Team Leader for the Older Persons Care Hub to oversee development, coordinate the clinical team, and deliver high-quality home-based assessments and short-term interventions.

You will lead stakeholder engagement, drive project implementation, supervise the nursing team, carry a clinical caseload, and ensure deliverables align with the Department of Health.
