Community Nurse - Sydney, NSW

We’re looking for someone who is compassionate, reliable, and confident working independently to help Santa Clara Home Care support seniors, empower people with disabilities, and deliver exceptional in-home care.

About The Role

- Provide personalised in-home nursing support, including clinical assessments, wound care, medication administration, social engagement, and emotional support.
- Support clients with home-based clinical tasks, such as health monitoring, care plan implementation, safety monitoring, appointments, and wellness advocacy.
- Deliver safe, high-quality care, including continuous health monitoring, medication management, clinical documentation, collaboration with families and health professionals, and identifying hazards to keep clients secure at home.

PAY Pay: $52.00 - $60.00 per hour for ordinary weekday hours, inclusive of the 25% casual loading and an above-award payment, plus 12% superannuation. Applicable evening, night, weekend, public-holiday and overtime rates are paid separately. A minimum two hours’ pay applies to each engagement or period of work in a broken shift.

About You

- Qualified and experienced, with current AHPRA registration as a Registered Nurse, police check, Professional Indemnity Insurance, First Aid/CPR, COVID-19 vaccination,



and at least 1 year of relevant community or aged care experience.
- Reliable, compassionate, and client-focused, with strong communication skills, good rapport-building, punctuality, and a willingness to learn and grow.
- Flexible and travel-ready, with a NSW driver’s licence, insured vehicle, mobile phone, and the ability to travel for client visits across Greater Sydney.
