Disability & Mental Health Support Worker — SIL & Community

About us
Health U Australia is a registered NDIS provider with years' experience across community health, mental health and disability. We support children, adolescents and adults in supported independent living (SIL) homes and out in the community with genuinely person-centred care.www.healthuau.com.auAbout the roleNo two participants are the same here, and no two weeks look the same. You might start the week doing personal care and meal prep in a SIL home, and end it supporting community access and appointments for a participant with a psychosocial disability.
We're looking for support workers who adapt — to different participants, plans, locations and shift types — without dropping the quality of care. If you need every shift to look the same, this isn't the role. If variety is what keeps you sharp, it is.
What you'll do
- Personal care: showering, dressing, toileting, continence care, transfers and manual handling
- Daily living support: meal preparation, medication prompting, light domestic and household tasks
- Community access: shopping, outings, activities and appointments
- Transport, short and long distance (company vehicle available for some shifts)
- Psychosocial and mental health support — building rapport, routine and independence
- Working to NDIS care plans and behaviour support plans across a varied caseload
- SIL day, evening and sleepover shifts on a fixed weekly roster set by Health U
What you'll bring
- Hands-on experience in disability, mental health and/or aged care — confident with personal care and manual handling (essential)
- Adaptability and reliability:
comfortable working with very different participants, and able to commit to your assigned weekly roster
- Australian driver's licence and a reliable, comprehensively insured vehicle; willing to travel between locations
- Live within a 25-minute drive of Ryde, Normanhurst or Hornsby
- NDIS Worker Screening Check and Working With Children Check (or willingness to obtain), plus current First Aid/CPR
- Strong English communication skills, patience, and a person-centred approach
- Cert III Individual Support, AIN/nursing background, or qualifications in mental health, counselling, social work or psychology are highly regarded
