About the role

As a Home Stretch Coach, you will work alongside young people aged 17.5 to 21 years to help them build independence, achieve their goals and strengthen their connections to family, culture, community and Country. This is a rewarding role where you will provide practical support, advocacy, mentoring, case coordination and life skills coaching to young people as they transition into adulthood.

Key responsibilities

- Develop strong, positive relationships with young people.
- Deliver culturally safe, trauma-informed and strengths-based support.
- Assist young people to access housing, education, training and employment opportunities.
- Support young people to develop life skills and independent living skills.
- Strengthen cultural identity, family relationships and community connections.
- Advocate on behalf of young people and coordinate service supports.
- Maintain accurate records, case notes and reports.
- Work collaboratively with the Department of Communities and local support services.

You are someone who





- Has experience working with young people and vulnerable communities.
- Understands trauma-informed and strengths-based practice.
- Is passionate about supporting Aboriginal young people to thrive.
- Has strong communication, advocacy and relationship-building skills.
- Can work independently while contributing positively to a team environment.

Requirements

- Relevant qualification in Community Services, Youth Work, Social Work or related field (or equivalent experience).
- Current C Class Driver's Licence.
- Working with Children Check.
- National Police Clearance.
- Ability to travel within the Pilbara region if required.
