We are currently seeking a Registered Nurse or Enrolled Nurse to join our team at Kedron Medical Centre. This is an exciting opportunity to play a key role in Chronic Disease Management, working closely with GPs and patients to improve health outcomes through preventative care and ongoing support.

About the Role
In this role, you will:
- Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
- Support patients in managing chronic health conditions and achieving positive health outcomes
- Conduct health assessments, including preventative health and chronic disease screening activities
- Work closely with GPs, specialists and the wider multidisciplinary healthcare team
- Provide patient education, coaching and ongoing support to encourage positive health behaviours
- Coordinate referrals to allied health professionals and external healthcare providers
- Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis where appropriate
- Utilise tools such as POLAR, PenCAT and HotDocs to identify eligible patients
- Maintain accurate clinical records and appointment administration
- Support preventative health initiatives and proactive patient outreach programs
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Previous experience in Chronic Disease Management highly regarded
- Experience in General Practice or Primary Healthcare preferred
- Strong communication and interpersonal skills
- Excellent organisational and time management abilities
- A proactive, patient-focused approach to care
- The ability to work both independently and collaboratively within a team
- A passion for preventative healthcare and improving patient outcomes
