About the Prospect

Are you an experienced Registered Nurse passionate about community aged care, clinical excellence, and empowering clients to live independently at home?

Finncare is seeking a dedicated Care Coordinator / Community Nurse to provide person-centred care coordination, high-quality community nursing, and clinical support to the wider Home Support team on a part-time basis.

What You’ll Be Doing

- Care Coordination: Comprehensive assessments and design tailored, goal-oriented care plans.

- Community Nursing: Providing excellent nursing care to our community clients e.g., wound care, continence management and health monitoring

- Clinical Support: Mentor, educate and support our direct-care team (Home Care Workers), fostering a culture of high performance and compassionate care.

- Education & Training: Deliver practical staff training in core clinical areas such as medication prompting, infection control and hand hygiene.

- Stakeholder Collaboration: Act as the point of contact for clients, families, GPs, allied health professionals, and internal rostering/management teams.

- Quality & Compliance: Maintain accurate records and reporting in accordance with the Aged Care Act and quality standards.

Essential Criteria:





- AHPRA Registration: Current, unrestricted registration as a Registered Nurse in Australia.

- Qualifications & Clearances:

- Current First Aid & CPR Certificates.

- Current National Police Check (or willingness to obtain).

- Current QLD Driver’s Licence and a reliable vehicle with comprehensive insurance.

- Care Coordination: Demonstrated experience in conducting comprehensive assessments and creating care plans in community settings.

- Leadership & Coaching: Experience in providing direction and support to Home Support staff.

- Communication & Tech Skills: Exceptional communication, stakeholder negotiation and analytical skills, with proficiency in digital client management systems and MS Office.

Desirable:

- Previous experience within Support at Home SaH and Commonwealth Home support Program CHSP

- Proficiency in the Finnish language (written and oral) is highly regarded.
