We have a fantastic, remote VR GP opportunity, providing structured chronic disease management for veterans and patients with complex, multi-system health needs.

This is not high-volume telehealth. You’ll provide ongoing care to patients you follow over time, working as part of an integrated Chronic Care Team across Medicare and DVA billing streams.

• $215/hr guaranteed for the first 3 months
• 70% of billings once your patient list is established
• 100% remote / work from home

• Flexible scheduling Monday to Friday
• Full administration, care coordination and allied-health support

The role will include:

• Telehealth consultations for chronic, complex and multi-system patients
• Chronic disease management reviews and care planning, including GP Management Plans and Team Care Arrangements
• Integrating pathology,



imaging and allied-health input into coordinated care plans
• Prescribing, referrals and investigation management within the care model
• Working collaboratively within a multidisciplinary Chronic Care Team

What you'll need:

• FRACGP or FACRRM & AHPRA registration with no conditions
• Demonstrated experience in chronic disease management and complex care coordination
• Veteran Health experience preferred
• 2 days per week preferred
• Confidence with investigation management

VR GPs subject to the 10-year moratorium who require DPA are welcome to apply
