Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health. Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.

We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.

About the Role

Omnia Community Health is seeking a compassionate and skilled Family Services Practitioner to join our Loddon Family Services team. This part-time, fixed term role supports children, young people and families through child-centred, strengths-based and culturally responsive practice.

You will provide early intervention, case management and intensive wraparound support across the Strengthening and Restoring Families streams, helping families build safety, stability and connection while reducing the need for statutory involvement.

In this role, you will work directly with vulnerable children and families to assess needs and risks, develop Family Action Plans, and deliver practical and therapeutic support through outreach, in-home and office-based work.

You will collaborate with Child Protection, The Orange Door,



education, health and community partners to provide coordinated, trauma-informed responses that promote child safety, wellbeing and family functioning.

You will apply relevant practice frameworks including MARAM, Safe and Together, information sharing schemes and culturally responsive approaches.

About You

You will bring experience working with vulnerable children and families with complex needs, along with an understanding of family violence, trauma, attachment and intergenerational disadvantage. You will be confident working independently, as part of a multidisciplinary team, and in partnership with families and services.

What we’re looking for

- A Bachelor qualification in Social Work, Psychology, Counselling or another human services related discipline, or equivalent qualification and relevant field experience, is required.
- Significant work experience in a relevant field working with vulnerable children and families with complex needs.
- You will also need a current Victorian Driver’s Licence and be able to meet employment screening requirements, including a National Criminal History Check, NDIS Worker Screening Check and Working with Children Check.
